National Disability Insurance Scheme (NDIS)
We can assist with application for approval and provision of services including functional capacity assessments, life skills development, Assistive Technology (AT) prescription and home modifications.

Aged care
Wellness and reablement focussed OT services under My Aged Care home care package provider referrals.
We are also able to provide cognitive capacity evidence reports.

Department of Veterans Affairs (DVA)
A wide variety of services including home safety assessments, falls prevention, equipment prescription and home modifications.

Private health
Direct referrals are accepted and may be claimable through your private health fund.
Centenary Occupational Therapy provides services by registered therapists experienced in working with the National Disability Insurance Scheme, Department of Veterans Affairs, My Aged Care and direct private referrals.
Clinicians are all current members of OT Australia and ARATA (Australian Rehabilitation & Assistive Technology Association).
Services are provided by home visits or telehealth only.
Home visits are available in the Brisbane western suburbs through to CBD.
Greater Brisbane, Ipswich and Toowoomba regions require additional negotiations, please see below.
Travel Regions
Centenary Suburbs
5–10 minutes - Westlake, Sinnamon Park, Jindalee, Mt Ommaney, Middle Park, Riverhills, Seventeen Mile Rocks.
Travel-fee free zone no travel charges apply for participants strictly located in these suburbs.
Brisbane West
10–20 minutes - Indooroopilly, Corinda, Graceville, Chelmer, Fig Tree Pocket, Chapel Hill
Local zone minimal travel charge applied.
South-West Brisbane / Ipswich
15–35 minutes- Forest Lake, Oxley, Inala, Goodna, Springfield Lakes
Standard travel charge applied includes return travel time.
Ipswich Central
25–45 minutes - Yamanto, Redbank, Bellbird Park, Churchill, West Ipswich
Standard travel rate applied; includes return travel time.
Lockyer Valley
60–75 minutes - Plainlands, Gatton, Helidon, Adare, Blanchview
Travel negotiated on request
Toowoomba Region
75–90 minutes - Centenary Heights, Rangeville, Glenvale, Wilsonton, Top Camp
Extended travel area – travel charges apply as per relevent guidelines; costs are shared between multiple visits where possible.
Long-distance service area – travel time and costs shared across multiple participants where possible and advance agreement has been obtained.
